Primary Care Home (PCH) models have been sweeping the nation, with more than 200 sites having emerged in the past three years since their introduction. Since then, the PCH model has had unprecedented success working collaboratively across their localities forging working partnerships across secondary, community and social care, whilst keeping primary care at the core.
Although, in a world where the determinates of its success relies on data, effective system integration, cross-sector collaboration at scale and patient engagement, how do professionals then achieve a sustainable balance between all these factors to execute such a complex model of care delivery? Additionally, following the emergence of STPs and ICSs, what impact will they have on the established PCH models?
This session will look at the achievements and challenges associated with PCH models amidst the changing health and social care landscape and will take a prospective look into the future as the transition towards greater integrated care gains momentum.
