The current lack of joined up care within the healthcare environment means that parents, individuals, carers, educational and healthcare professionals often have the same information but are unable to share it. Created by parent carer Nicola Murgatroyd, and developed with the user and their family at the centre of the system, MyLiferaft provides a solution to this challenge of keeping healthcare information current and accurate.
Accessed using a laptop, tablet or phone, MyLiferaft is a secure online tool where parents - or people with additional needs themselves - can store, manage and share all aspects of their health, care and well-being information. MyLiferaft helps coordinate effective care provision across multiple health and social care services, bringing huge benefits to the individual and their family by:
MyLiferaft starts with the individual or family unit, putting them in control of their healthcare provision. It provides one place to store and manage all aspects of care information, and allows the person to choose what information is shared and with whom.
The tools and resources available within MyLiferaft have been developed with input from those using the system to enable the individual, their family and their care network to track, collate and share care and wellbeing information. No other application brings health and social care together in a single place; managed and owned by the individual at the centre of the care circle and/or those who are caring for them.
Technology Enabled Care Services (TECS) is becoming the accepted description for a range of health and care technologies such as telecare, environmental controls, telehealth, mHealth and telemedicine. TECS is not about a device or a thing, but about using technology as a tool to help improve lives through enabling people to ‘live well and keep well’. MyLiferaft helps TEC become more personal in that it gives the individual more control around their own care and record preferences to reflect how they feel and want to live; it enables families to see and understand where TECS really can make a difference to their lives
MyLiferaft sits at the heart of flexible and bespoke packages of technology led support. For the first time, MyLiferaft introduces a holistic approach to care by providing one true source of information as all the formal health and social care details can be integrated with the informal ‘personal’ care and well-being information. The platform is designed to be compatible with existing and emerging systems so that it can integrate with other products to bring about complementary and supportive technological solutions to the health and social care sector. MyLiferaft compliments many other apps and devices through the consolidation of data which in turn, benefits the individual, the care team, and the health and social care services.
